  • The fence around the playground located close to the facility has a broken top piece of fence near the chicken coop/storage. The back playground has a fence piece not tied into the remaining fence.
  • Director has an expired credential. Expired on 5/27/23.
  • Licensing observed that Director and one staff, PB have an expired PDL.
  • Licensing observed one staff, HM after 90 days of hire, not current on ELCCT or have documentation of CPR/FA.
  • Licensing observed one child without a child information sheet, without immunization records on file and did not have medication permission from parent for Desitin diaper cream.
  • Licensing observed all personnel not having daily attendance on file.

  • Licensing observed physical environment checklist not completed since 4/28/22.
  • Personnel could not provide transportation itinerary for van to licensing.

  • (Serious) Licensing observed hazards items accessible in bathroom in the infant room in unlocked cabinet. Hazards chemicals were Lysol disinfectant spray, Lysol disinfectant wipes. Licensing observed hazards items accessible in bathroom in the middle room in unlocked cabinet. Hazards chemical was a bottle of liquid Clorox bleach.
  • (Serious) Licensing observed prescription medication accessible in bathroom located in the infant room in 2 different locations. Under the sink cabinet unlocked and in 3 tiered cabinet unlocked.
  • Licensing observed outdoor slide broken with sharp edges and numerous outdoor play equipment with broken wheels, pedals and mixing parts.
  • Licensing observed a 2 year-old child with a bottle in the toddler room during nap time.

However, additional hazards were accessible located underneath the sink and doors do not have any child proof latches. Hazard were chemicals; half gallon bottle of bleach, bathroom cleaner.
  • Serious- Licensing observed a staff's purse in the bathroom near the infant room that was sitting on the sink and opened which prescription medication was visible and accessible.
  • Serious- Licensing observed upon arrival director sitting at desk. Director stated she was working on paper work. Licensing observed that the 3 and 4 year old group was out of ratio. Director was need in class to meet ratios.
  • Licensing observed one freezer without a thermometer.
  • Licensing observed both bathrooms did not have paper towels or individual towels for hand drying.
  • Licensing observed no menu or children's allegories posted in kitchen.
  • Licensing observed in the kitchen the floor vent has slates missing and causes a hole in floor.
  • Repeated- Licensing observed one personnel, employed since 2018, OPDL has been expired since 4/18/21.
  • Licensing observed bottles in infant room without names labeled.
  • Licensing observed fire extinguisher have not been inspected and tagged according to requirements.

  • Serious- Licensing observed 35 children in care at program. Programs maximum capacity is 30.
  • Serious- Licensing observed children ages 2 years and older (1 two-year-old) with 14 children present with only 1 staff member.
  • Serious-Licensing observed hazards items accessible in both bathroom. In bathroom near infant room, cleaning supplies (Scrubbing bubbles bathroom cleaner) were sitting on the cabinet. The bathroom in the hallway the cabinets underneath the sink did not have any child proof latches and allowed the doors to be able to be opened and a gallon bottle of bleach was under the sink. In the laundry room cleaners were accessible to children. Children have to walk through the laundry room to go outside.
  • Licensing observed bottles in infant room without names labeled. Licensing asked personnel if they label bottles and personnel stated no.
