Non Compliance Observed Licensing viewed 4 children's records. Two (2) children did not have shot records on file. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Director is signing up to get shot records from OSIIS. Director will request paper copy of immunizations till you are able to receive children's immunization records on OSIIS. In the future the program will maintain licensing requirements. Regulation Description Immunization records. An immunization record or exemption is obtained prior to the first day of attendance and is updated when the child receives additional vaccines. Infection control
Non Compliance Observed Licensing observed that personnel has not obtained infection control training since 5/10/22. Licensing discusses, consulted and provide free training for infection control at monitoring visit on 1/18/24. Licensing documented in 1/18/24 monitoring visit discussion. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct No acceptable Plan of Correction at this time, over 90 days expired. Director will have all personnel enroll and complete this week. In the future, director will maintain licensing requirements. Regulation Description Infection control.
