Moving forward, all Higher risk activities will be monitored closer with the ratio chart found on page 113 of the Licensing Requirements book to ensure that the safety and well-being of each child is met. A compliance course was taught to all staff on 12/23/2024 by our Licensing Specialist.

Regulation Description

340:110-3-290(c) - Ratios and supervision. Teaching personnel required for ratios are present with the children, including when the activity is conducted by contracted non-personnel. Additional personnel may be required, depending on the activity risk-level, to ensure proper supervision and care of all children.

Plan to Correct

Moving forward, all personnel and staff are aware parent permission slip is needed in writing before a child can participate in a high risk activity. This was reiterated to staff by DHS during a compliance course and an extra safety course the CDC required teachers take from OKregistry.

Regulation Description

340:110-3-290(e) - Higher risk activity plan and parent permission. Prior to the activity, the program provides the plan to parents and obtains their permission. Documentation is maintained per OAC 340:110-3-281.4(b).

Plan to Correct

A plan of action will be written stating that when children participate in a higher risk activity parents will have a previous knowledge of where, when, who, and what the activity is as well as a signing permission slip stating 'yes or no' on if their child(ren) can be included in the activity.

340:110-3-281.4(b)(13) - Higher risk activity permission and plan. A plan is provided to parents and signed parent permission is obtained prior to the activity. Documentation of both is maintained for at least 12 months from the activity. Permission may be for each time the child participates or a general permission for each activity type, such as swimming, may be given for 120-calendar days or less at a time. All teachers were made aware that any bump, bruise, or cut should not only be put into procare, but a simple message should be sent to parents at the time of the incident to ensure parents are well informed of all accidents/injuries.

Plan to Correct

Moving forward, all staff will have a refreshed learning of how to properly supervise children by taking an online OKregistry course. Staff will be recommended to radio for assistance if/when a second set of hand and/or eyes would be necessary and helpful. All staff are being reminded to be aware of the children's movements at all times and to make note of any possible injury to Director and parents via the procare app.

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Plan to Correct

Moving forward all staff will be required to take a refresher course within 90 days or face a write up and possible probationary period. Staff will be reminded to report all questionable interactions to Director and that staff is being monitored more clearly. Any staff being aggressive towards a child in any way will be asked to leave for the day while their employment is renewed for continuation. The teacher responsible is no longer employed with the CDC.

Plan to Correct

All staff will be reminded that children do not cry for no reason and any child who seems visibly upset needs to be checked over physically and emotionally when able to speak. At the next staff meeting, teachers will be reminded to use basic first aid and TLC when a child has injured themselves. Staff will be required to take a requirements course with our licensing specialist to ensure they are aware of what is expected of them in/out of the classroom. Teacher responsible is no longer employed at the CDC.

340:110-3-284(c)(2)(E) - work with children without physical, psychological, or emotional punishment, mistreatment, or abuse.

Plan to Correct

Teacher was not allowed to continue employment. We are training teachers more each day to understand and comply with DHS rules and regulations. Licensing books are being placed in each classroom for a guide.

Use of belts for any reason, in our facility, are prohibited. The waist belts and leashes were thrown in the trash. For transport to different areas of the CDC, we now utilize a buggy system, walking rope for students to hold onto when traveling. (These are not attached to them) We also have worked on having additional volunteers available to assist when our oldest children in the CDC travel. The utilize a line up and follow the leader system. Regarding physical or disciplinary action with belts, there are no belts to utilize in our facility. They have been tossed in the trash.

Plan to Correct

This employee has been terminated from employment at our CDC for this very action. This style of discipline or behavior is unacceptable. The way that our teachers handle discipline is by redirection, talking with children about their actions, we utilize calm down time by allowing children to go to a cozy corner in each classroom to manage emotions or feelings. We do sit out time from play to discuss our feelings and actions if the child is age 2+. We also take advice from parents on how to better manage outbursts and meltdowns with their own child at home. We keep in communication with our parents when children are struggling during the day with their own emotions or behavior through our parent data system. We will also have another teacher assist if there is availability and we feel that the child will better respond to another face in the classroom. We will never result in physical harm to a child.

Teachers have all had an individualized meeting with the Director on this matter. We are doing proper protocol on timely diaper changes in the classrooms. Having an assistant teacher to assist in the classroom has also helped in this matter. Reminders on the walls for the baby room of the time the child was last changed it utilized. Setting reminders on phones, watches or tablets to keep track on the time of last changing diapers is also utilized as needed. The Director does check in with teachers daily to keep them mindful of keeping up with these daily activities.

Plan to Correct

The current Director is in the office daily, she keeps track of employees and employees in return have a radio on hand they can utilize if assistance is ever needed. If the Director is ever away from her desk, we have her second in command, Mrs. Wendy Adams, to assist. As well as a chain of command for floor teacher/persons. Security System that footage was caught on, has been moved to the Directors office and is utilized for the Director and assistant to have a better and close eye on teachers/classrooms and the type of assistance that is needed.
