Licensing observed daily records were not available in the child's classroom for 1-year-olds and younger on site at the time of visit.
Licensing observed the outside play area did not meet the outdoor play equipment per appendix mm' no interest areas available, no equipment available for children under the age of 2.

When licensing arrived at 11:01 am, it was observed that an open container of milk and an open container of potatoes where left on the counter in the kitchen.
Program personnel did not have control of children when bringing back to the facility; 4 children ran ahead and program personnel were unable to keep up with them.
When licensing arrived at the program it was observed the outside play area did not have outside shade accessible to the children; the children were outdoors.
When licensing arrived the director was in the program counting in ratio and preparing lunch in the kitchen.
Licensing observed 2 staff not having maintained current CPR/FA within 2 years at time of visit. One staff was left alone with children on site with 6 children without current CPR/FA at time of visit.

Program transported children 5 years and older this date and does not have any booster seats present in the vehicle for children under 8 years old.

Licensing observed a crib was next to a wall with an outlet, no faceplate, and a Samsung charger plugged into it. The charger was sticking into the crib slats, with the charging cord bending into crib slats making it easy for the one year old in side the crib to pull the cord out of the outlet.

Supervision -Staff are leaving children unattended in a classroom to answer the front door.
Plan to Correct
Plan of correction-I will always ensure that we have enough staff to cover each and every classroom at all times. I will continue to have ongoing staff training and retaining. I will ensure during staff training and orientation to emphasize on safety issues including supervision. I will implement active supervision practices. I will continue to take proactive measures and ongoing trainings.

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Nutrition- Program is not serving milk or fruit with lunch.

Regulation Description
340:110-3-298(b)(1) - Meals and snacks. Meals and snacks are required to meet the current Child and Adult Care Food Program (CACFP) guidelines, per Oklahoma State Department of Education (OSDE), with the exception of infants, per (f) of this Section.

340:110-3-288(c)(1)(F) - restraining or tying to restrict a child's movement by any means other than holding. Holding is used only as long as necessary for the child to regain self-control.

Rest time - Program is forcing children to remain on cots after resting.

340:110-3-296(e)(4) - After resting. Once children have rested, they are not forced to remain on the rest equipment and are provided quiet activities until rest time is over.

Learning Program Principles - Program is allowing screen time for infants and one-year olds.
Plan to Correct
Plan of correction- I have and will follow my plan of correction which includes strictly exclude screen time for infants and 1-year olds. I will ensure that we have interactive screen-free activities that promote critical language skills, emotional regulation, socialization, and creative problem-solving. I will also ensure establishing healthy boundaries which will help the infants and 1-year olds to thrive as they grow.
