A teacher in the older age classroom, left their classroom without supervision to answer the door for child care services workers. The children were all asleep in the classroom.
In the younger classroom the diaper trash can did not have a lid.
There were cleaning supplies accessible in two restrooms used by children. They were on the counters, on the toilet backs, and under the cabinets with no locks. There was cleaning sprays accessible on two shelves in the older classroom.
There is equipment that was placed in the grass with no impact material with the depth of 6 inches in the use zone. This included one single axis swing set and one climber with slide. One swing set was placed in the sand but it's use zone based on chain length was not fully covered in impact material
Two hand washing sinks were at 140 degrees Fahrenheit.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.
