Licensing observed that 1 year old room was out of ration with 9 children and 1 staff member. # year old room was out of ratio with 15 children and one teacher (11 of the children were 3 year old's- There should be no more than 6 three year olds per teacher.) Reverting to single age group for this classroom, 3 years olds ratio is 1:12. Second class room with 3 year olds and older had 16 children to 1 teacher.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Complaint:
Supervision - an aggressive child injured other children due to lack of supervision.
Plan to Correct
Teachers will routinely walk the classroom and between projects they will walk through the room. A member of the administration team will walk through the school-age classroom every 3o minutes to check in on the class. Teachers will partake in in-class training with administration on supervision and conflict resolution. On May 14, 2026, we will be holding a meeting to go over teachers expectations, following DHS regulations, and future training procedures, such as in-class training with administrators.

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Reporting - Parents are not receiving incident reports or being informed when and why injuries occur.
Plan to Correct
Injury and incident reports will be completed and signed at the time of injury/incident by teacher and administrator. Parents will be notified by administrator at time of pick up. Parents will be called and notified of any injury to the head, at the time of injury. The director will discuss with personnel how to ensure that all incident and injury reports are completed correctly. In the future, parents will be notified as required.

Ratios (SERIOUS) - rooms have too many children in it making them out of ratio
Plan to Correct
Notice to Comply states the following: Director stated that she came in and opened another classroom and started putting kids in there. Also they will be letting parents know that they can't drop kids off if classroom is full. Terri Beier, the owner, came in and was looking over the staff and children's schedules to see where they need to increase staff and when. Director will also check into phone line rolling over to another number if mainline doesn't answer. Also will give training to personnel on how to handle situation with parents and ratios. Director has typed up a letter to send to parents asking if they are going to bring their children in at a different time than scheduled to please inform child care before they do so. In the future, director will maintain proper ratio by ensuring enough staff is present.

340:110-3-286(a) - Ratios. Ratios are the number of teaching personnel required to be physically present with a child or group of children, per Oklahoma Administrative Code (OAC) 340 Appendix GG - Ratios and Group Sizes. Only teaching personnel count in ratios, unless requirements specifically state otherwise. The program is required to maintain ratios at all times.

Notice to Comply completed by Responsible Entity will make sure teachers are checking hourly. Spoke with teachers abut checking and changing every hour as required.

Notice to Comply completed by Responsible Entity will make sure children in pullups are going to bathroom every 30 minutes to 1 hour. Talked to teachers about the importance children being taken to bathroom hourly as required. But not only that to help parents get children potty trained.

6)Learning Program Principles - School age classroom is a mad house with no structure and no control of children.
Plan to Correct
Director stated a member of management will be present in the room during transitions to help establish routine. Going forward a member of management will be in the school age room 2 during drop off and transition times until the room can be singled all. The member of management will be present to help establish a routine in the classroom and to get the kids in a regulated schedule. We do plan on hiring a third person to assist in the room. but do not have exact date as to when a new member of staff will be hired at this time. Going forward there will always be a member of management present during drop off from school and transition times to keep the room calm and on track with routine.
