Plan to Correct

Upon becoming aware of the serious non-compliance, the previous director immediately self-reported the incident to our DHS licensing specialist after being notified by the parents. The staff member involved was immediately removed from the infant classroom and informed that she would not be returning to the infant room indefinitely. To prevent future occurrences, a staff meeting was held on May 21, 2026, to review all DHS safe sleep requirements and safe sleep protocol with all staff members. Expectations for compliance were clearly communicated, and staff were given the opportunity to ask questions regarding proper procedures. Additionally, safe sleep reminder signs have been posted above every crib in both infant classrooms to provide continuous visual reminders of required safe sleep practices. As the new Director, I conduct routine checks throughout the day in all infant classrooms to ensure safe sleep practices are being consistently followed. During these checks, l verify that all infants are placed on their backs to sleep, cribs remain free of prohibited items, and staff are adhering to all DHS safe sleep requirements. I will continue ongoing monitoring of infant classrooms and provide additional training as needed to ensure all safe sleep policies and procedures are consistently followed and maintained in compliance with DHS regulations.

Regulation Description

340:110-3-296(b)(2)(C)(i) - Infants are placed on their back for sleeping, unless there is a medical reason an infant should not sleep in this position as documented per OAC 340:110-3-281.4(b).

Plan to Correct

We will make sure that all the children in the center will have a medication forms for diaper cream with parent signature. In the future we will make sure that we will get the permission forms before administrating diaper cream.
