  • Primary Caregiver's PDL expired on 12/5/2024 due to lack of professional development hours.
  • 6 hours of 20 hours required of professional development is completed for this PDL renewal.
  • One year old child was asleep in a play pen with a bottle that had milk inside it.
  • There was hairspray and hair cream that had a warning message to keep away from children on the packaging within reach of children on the bathroom sink.
  • The living room does not allow space for the children to play due to furniture, a treadmill, two end tables, and an approximate 4 ft pile of donations, which included clothes, toys, and unassembled shelving. The dining area which is required for licensed space dining room table is covered with laundry.
  • One family did not complete the compliance file notification form in the past 12 months.
  • Caregivers' attendance records did not indicate the hours worked on the documentation.
  • On 4/11/2025, a drop in child did not have the arrival and departure times documented.

  • Upon arrival there were 4 infants and only 3 playpens set up. Upon review of licensing one play pen mattress pad board was found to be broken. Meaning that only 2 playpens were available for use on this date.
  • One child was missing the first page of the Child information form being used by the program.
  • One child was missing the immunization record.
  • The living room and kitchen were cluttered with bagged food on the floor of the kitchen, laundry on the couch, and surfaces covered with paper, books, and other items.
  • Primary Caregiver and Assistant caregiver on-site and counting in ratio did not have current CPR and First Aid training. Expired on 12/28/2024.

  • Trampoline attached to swing set was accessible to children outside. The netting to prevent access was no longer in place and had fallen down.

  • Living room was cluttered and not easily accessible for moving around without moving clothes, toys, and other equipment. Infant sleep equipment was not accessible due to couch. The outdoor play space was clutter with trash and non-play items.
  • There was hair detangle hair spray that requires to be kept out of reach of children on a living room table within reach of children.
  • Infant was fed baby food and bottle in saucer and child's hands were not washed prior to eating.
  • Towels were not accessible to children due to container being empty. New towels were still wrapped in plastic..
  • Green beans in the fridge were uncovered. Cake Balls were also uncovered in fridge.
  • Surface where infant eats was still dirty from morning meal. The provider is using the excersaucer in living room.
  • Children cots were in the play space and being used for children for play during visit. Infant playpen where infant was standing awake in had crumbs and trash under the pad.
  • Under the pad in the infant sleep equipment being used at beginning of visit there was a play necklace and pieces of trash from food bags. There was multiple items such as plastic bag, books, and papers that were accessible to standing infant to be pulled into playpen.
  • The diapering changing pad was covered with other items and not usable. Provider is changing diapers on couch with a blanket.
  • Primary Caregiver does not have current CPR and First aid.
  • Primary Caregiver has not completed 15 full hours from year 8/2020-2021. Only 10 hours. 15 hours were also due for year 8/2021-2022. 20 hours are currently due in full. Third documentation of this non-compliance in 12 months.
  • Assistant Caregiver has 7 hours of professional development completed from 1/2021-1/2022.
