Licensing will email Program Director the New Enrollment Checklist which can be used to ensure all required documents are provided at time of enrollment. Regulation Description Immunization records. Non Compliance Observed Cubbies/Bookshelves were broken with sharp edges accessible to children. The Program will complete a Physical Environment Checklist for each classroom once a month. Non Compliance Observed Pads in play pens (yards) appeared too small for the play pen they were in. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct The two play pens were removed from the classrooms. Assistant Director will discuss with Program Director about purchasing cribs to replace the play pens. Program will inspect the play pens weekly to ensure they're clean & the pads fit properly. Non Compliance Observed Carpet in Infant Class 1 was dirty. Dried food & formula stuck in carpet. Rugs in Classes 3 & 4 were stained & had food particles on them. Classes 3 & 4 also had a musty, unpleasant smell. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Program will have the carpet cleaners come in to clean the carpets in all classrooms. Program will purchase new rugs for the classrooms that have stained, old rugs. The Program will train their staff where the children's designated eating/drinking areas are in the classrooms. Staff will also be reminded that they are supposed to clean their classrooms at the end of each day. Management will ensure that this is done daily. Regulation Description The facility and vehicles are clean, sanitary, and free of clutter and litter, including paper accumulation.
