Non Compliance Observed Licensing observed three (3) new personnel employed at program since, May 24, Aug 24 and Nov 24 that a personnel information form was not submitted to licensing. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Director will ensure that personnel forms are submitted as soon as background checks are completed from OBI. Director will also inform AD that AD can email licensing new personnel name and employment date and program can upload personnel form on CCMASS. In the future the program will maintain requirements. Oklahoma Professional Development Ladder
Non Compliance Observed Licensing observed on the okregistry that Master Teacher, KC, has an expired PDL. PDL expired on 1/14/25. PDL was submitted for renewal on 1/13/25. Training hours was completed on: 6/25/24 - 4 hours; 1/7/25 - 5 hours; 1/12/25 - 14 hours; 1/13/25 - 10 hours and Master Teacher, EW, has an expired PDL. PDL expired on 1/24/25. PDL was submitted for renewal on 1/15/25. Training hours was completed on: 20 hours taken 1/15/25. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct The director will have personnel have training hours completed 60 days prior to PDL expiration and personnel will submit PDL application for renewal no later than 60 days. Assistant teacher is not on registry, program did not have CPR/FA on file for personnel, and personnel did not have a copy of CPR/FA.
