Executive Director provided new training to the team in September regarding the school allergy plan. School will complete a follow up allergy training with both RM and ED on 9/30 with one-on-one follow up with each staff member the week of 9/28. Allergy plans will be updated as new allergies are shared and monthly by DQE in all classrooms.

Plan to Correct

We immediately reviewed the circumstances of the injury. We reviewed active supervision with all staff, including continuous sight and sound supervision, positioning and prompt intervention when needed. Provided documented retraining to involved staff on supervision, timely reporting and positioning. We are requiring teachers to maintain accurate child counts and use established transitions. Leadership will reinforce expectations during daily classroom check-ins twice per day with manager signatures. The leadership team will conduct and document unannounced supervision observations across classrooms. Maintain training records, observation documentation and corrective action records. The Executive Director will review trends and adjust support as needed.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Plan to Correct

The Executive Director will review staffing, schedule and ratios throughout the day and make immediate changes before any staff members leave a class or children move to a new class. Leadership will also conduct ratio checks throughout the day to verify compliance. Any identified concerns will be corrected immediately and documented. Any staff involved will be followed up with. Staffing schedules will continue to be reviewed and adjusted as needed to accommodate breaks, arrivals, departures, or as staffing needs change.

Regulation Description

340:110-3-286(a) - Ratios. Ratios are the number of teaching personnel required to be physically present with a child or group of children, per Oklahoma Administrative Code (OAC) 340 Appendix GG - Ratios and Group Sizes. Only teaching personnel count in ratios, unless requirements specifically state otherwise. The program is required to maintain ratios at all times.

Plan to Correct

Upon receiving notification to the finding, CDLC of Oklahoma City immediately reviewing the incident. The employee involved is no longer employed and will have no further contact with children in our care. To ensure continued compliance and reinforce expectations, all current staff completed mandatory retraining covering positive child guidance, appropriate child teacher interactions, mandated reporting practices, and Oklahoma licensing requirement 340.1 10-3-284 (c)(2)(e). Center leadership will continue to conduct ongoing classroom observations to ensure appropriate practices are consistently followed. Any concerns will be addressed and documented properly. Regional Manager will review training and center leadership will continue to ensure the center team follows state and company expectations.
