Re-educated all staff on proper diaper changing procedure, and updated procedure to include using safety buckles with each diaper change, soft landing mat placed next to changing tables 10/8/24-10/9/24. Requirements training completed with staff by OHS personnel 10/28/2024.

340:110-3-297(a)(2)(B) - per to Oklahoma Administrative Code (OAC) 340 Appendix KK – Diaper Changing Procedures. The procedures are posted, per OAC 340:110-3-281.1(h), and documentation is maintained, per OAC 340:110-3-281.4(b).

Sent for completion. POC completed on 11/20/24: All staff was re-educated on policy to report any fall/injury/ incidents to person in charge immediately. Person in charge is then responsible for contacting parents immediately 10/8/24-10/9/24. New procedure: person in charge is required to round to each room at a minimum of hourly to verify/check for any injury/incidents that could have occurred, and to follow up/take action if needed 10/10/24. Requirements training completed with staff by OHS personnel 10/28/24. Re-educated all staff to call out via walkie to person in charge for any injury/incident/fall that occurs for parents to be called10/8/24-10/9/24.

POC completed on 11/20/24: Re-educated all staff on 10/8/24 Per policy, staff in charge are to be made aware immediately of any injury/incidents so they can contact and make parent aware by phone right away. Child is attended to by teacher, while the person in charge contacts the parent. Person in charge will also direct other staff as needed pending situation. Teacher will then take photos, and complete incident report. Pictures and incident report are uploaded to bright wheel. Incident report is then filed. Requirements training completed with staff by DHS personnel 10/28/24.

Sent for completion. POC completed on 11/20/24: Teacher removed from any classrooms duties 10/08/24. Teacher was re-assigned job, with no direct childcare contact. All staff re-educated of one hand must remain on child at all times during diaper changing procedure. New updated policy implemented: safety buckles to be utilized every time on the changing table. Soft landing mat placed next to changing table. Updated diaper changing policy to reflect these changes 10/8/24-10/9/24. Requirements training completed with staff by OHS personnel 10/28/24.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
