Add clear incident-reporting steps to SOPs and employee handbook and create/post a classroom poster showing how to complete an incident report (including steps: fill out report, place in binder, notify director, escalate to licensing as required) Mandatory training on injury/incident reporting and ethical responsibilities quarterly refresher training.
