  • The door to the first classroom was not appropriately installed, causing the door to be loose off the opening and hanging onto the hallway, where children and staff walk through.
  • The gate at the playground fence has broken off the hinges. The gate cannot be closed while children are playing.

  • One of the classrooms at the program was out of ratio when Licensing arrived, having 17 children and two teachers. Six of the 17 children were one year olds.
  • Licensing could not verify that the fire extinguishers at the facility were inspected and tagged by a state licensed authority within the last 12 months
  • One of the program's teachers has been employed at the program since October of 2025 and has incomplete results.
