A hole in the wall was observed in the 3 year old classroom by the office door. When asked the hole has been there for a week.

Plan to Correct
Notice to Comply received. On 05/19/26, we had a meeting about leaving the classroom and being out of ratio per classrooms. We discussed how important it is to be in ratio and everyone understood. Training will emphasize that active supervision requires staff to be physically present, positioned strategically to see all children, and fully aware of the required ratios for their specific age group at all times. The Director and Owners will conduct random, unannounced "ratio sweeps" throughout the day, physically counting children and staff in each room to guarantee continuous compliance.
Regulation Description
340:110-3-286(a) - Ratios. Ratios are the number of teaching personnel required to be physically present with a child or group of children, per Oklahoma Administrative Code (OAC) 340 Appendix GG - Ratios and Group Sizes. Only teaching personnel count in ratios, unless requirements specifically state otherwise. The program is required to maintain ratios at all times.
