Plan to Correct

Plan of correction: Will make sure teachers are walking around and paying attention to all the children. Started a buddy system that when a child needs to come in to the program to go the bathroom or get a drink, a teacher outside will messenge the director and /or staff up front who will go the back door to let the children in and there will be two children at a time. Have put water jugs and cups outside so the children do not have to come inside as often. The staff will stay with children while they are at the bathroom and the staff will walk back to their designated spot with their class. The staff will let the teacher know that those children are back.

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Plan to Correct

Separate logs for each vehicle used for transport and will name each child for the vehicle they will be riding in. The driver and staff's name will be written on the same log. The children are numbered on the log with their full name. The teacher will do role call by saying the child's full name, the child will respond here and raise their hand. This will be done when the children get on the bus and again when the children get off the bus.

We will make sure there is enough teaching staff for the number of school age children whether the children are on or off-site at all times.

Regulation Description

340:110-3-286(a) - Ratios. Ratios are the number of teaching personnel required to be physically present with a child or group of children, per Oklahoma Administrative Code (OAC) 340 Appendix GG - Ratios and Group Sizes. Only teaching personnel count in ratios, unless requirements specifically state otherwise. The program is required to maintain ratios at all times.

Plan to Correct

Update the transportation attendance log to show the child's first and last name. The time in and out for each child is documented each time the child gets on or off the vehicle. This is completed separately for each vehicle when children are being transported. The teacher(s) and driver's names will be on the logs as well. The teacher and driver will sign the log each they reach destination.
