Wooden platform around the base of a large tree in play area had holes, big enough for a child's foot could fit in and get caught .and fall or get cut, Sharp edges from broken wood was noted that could poke a child, platform was not study when stood on. pieces missing and this platgorm could be pulled down with not much effort. Screws were protruding from wooden boarder in the play area. Worker counted 3 that could catch on a child and cause harm.

outdoor play area contains a wooden wrap around bench area that wraps around large tree. The bottom of the small deck/ bench is rotting with numerous screws /nails exposed and rotten wood is not sturdy.
outdoor play area had dirty/ broken equipment as well as lots of trash in the play area.
Chain link fence around play area has gaps and broken frame around top.
There was no insurance exception posted.
No posting to list personnel in charge.
Insurance exception forms not completed with parents. Compliance forms not completed annually.
