- The one year old classroom had diapers in a trashcan without a tight fitting lid. This trashcan was located in the classroom's restroom.
- On the toddler playground there was equipment measuring 32 inches in height. Impact material measured less than 6 inches. On the North side playground the impact material measured 5 inches around the large climbers.

Records "Teacher did not complete two methods to check van after parking to ensure that children were not left in vehicle. A child was left sleeping in the van and locked in for 20 minutes.
