340:110-3-297(a)(2)(B) - per to Oklahoma Administrative Code (OAC) 340 Appendix KK – Diaper Changing Procedures. The procedures are posted, per OAC 340:110-3-281.1(h), and documentation is maintained, per OAC 340:110-3-281.4(b).

Plan to Correct

New Supervision steps of action and training with improvement plans for the safety and well being of the children will be implemented. Date 9/27/2024. This will ensure no child will be left behind without supervision. The adult transporting children will ensure the new steps of action are being completed with exiting the van and will be completed daily, insuring the safety and supervision at all times of the children in our care. 1. The driver will visually check seat by seat to make sure no children are left behind sleeping or unattended on the van. 2. Face to Name check once off the van before entering the building-Checking off on transportation form when completed. 3. Face to Name check when inside the building for aftercare, insuring all are inside and safe. The sub driver has been removed from transportation until further ongoing annual training on Supervision and transportation has been completed along with the probation period. Sub driver was written up and put on a 90 day probation period and given supervision requirements to meet daily with trainings. Date due by 12/24/2024. An additional extra key to the building for vans has been copied and placed with extra van keys inside of the school age building. Date completed 9/27/2024.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Plan to Correct

Once arriving to the center for after care and prior to leaving the van after transportation, the driver will; 1. Visually check inside of the van looking in all seats making sure all children are unloaded using the roster check list. Date and Initials will be applied as the driver does the safety check. This routine and checklist roster will be followed daily. 2. Face to name check before entering the building. 3. Once arriving inside the center for after care, the driver will use the roster and do a face to name count when signing them in for after care inside the building to ensure all are inside safe. Dated 9/27/2024. We will have the first refresher training on transportation safety and supervision steps of action, which will be required on-going annually. New procedures following all of the above steps have been written and will be followed and signed by all drivers to ensure the safety and well being of children in our care. Once inside of the building for after care and three methods of safety have been met, children will be led to continue our routine of after care using the roster checklist for staff/child ratio. In addition we will ask for Resource and referral to schedule and complete a supervision training. Dated 11/5/2024.

Regulation Description

340:110-3-281.2(d)(6) - Child passenger check. Before leaving the vehicle and in addition to the transportation attendance, the driver or last personnel in the vehicle documents completion of at least two methods of inspecting all areas of the vehicle for any remaining children to ensure no children are left on the vehicle.
