  • outlet observed in infant room has not having covers and is accessible to children
  • all mattress's did not have a form-fitting material on them
  • formula bottles were still being used after one hour of prepared
  • facility was observed with having lots of clutter, paper, soil, dirt, and other debri accumulation in every classroom.
  • baby food provided by parents was not labeled with child's full name
  • refrigerators and freezers contained spilt liquids and food debris.
  • standing liquid observed in mop bucket
  • multiple cabinets had cleaning materials underneath with no locks on cabinets and accessible to children
  • trash container observed with no liner and trash had not been removed in sometime
  • multiple areas in the center with rodent feces observed. Including the kitchen, pantry, bathrooms, and classrooms.
  • One three year old child in classroom with no staff.
  • Three year old child with no staff. Supervision was not being maintained for this child.
  • Multiple staff were unable to determine ages of children in their care.
  • Multiple hazards to children's health and safety were being ignored by all staff.
