Plan to Correct

Plan of correction: The teacher who was responsible for those kids has been let go, We have printed roll call sheets and all teachers are to do name to face head counts periodically when they are in the playroom or taking the kids outside, they are to walk around observing the kids and doing several name to face head counts. In the future during training they are supposed to train the new staff to-do the same. We conducted a emergency staff meeting stating about what happened and what we needed to do and talked about proper supervising of all the kids. They then had to sign that they were there for that staff meeting. Since the incident we have had 2 teachers in that classroom to ensure proper supervision is happening, Their has also been an alarm put on our door leading to outside and We check it everyday before we get the school kids and after getting the school kids. Then finally me as the director will observe that they are all doing this and observing to make sure all staff on a regular basis ls engaging with the children and doing head counts, and if they don't l will let them know that this will result in counseling staff, a write up or termination.

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

We have had a meeting with all personnel that if an incident that serious happens again they will wright an incident report, a parent and teacher will sign it and then we will also call that parent to let them know immediately. In the future we will make sure all DHS guidelines are followed if they are not depending on the matter they will be wrote up or fired.

We have talked to and had a staff meeting with all teachers about the guidelines and DHS policies & if a child leaves our facility, we are to make an incident report with parent and staff signature& notify by calling them immediately. They also know to head counts periodically.

The director will make sure all personnel information sheets will be send within 2 weeks of them being hired or with in 2 weeks of them being rehired to our facility. As they are being rehired we make sure we go over DHS policies and guildlines.
