Additional Non-Compliance Found During Investigation: SERIOUS: Supervision - Teacher did not have knowledge regarding the whereabouts of a child in the assigned group while off-premises.
Plan to Correct
On school-aged field trips, we typically have no more than 7 students per teacher in a group. We will discuss with teachers any students who may need additional guidance and supervision during field trips. Those students will be placed in either a group with less than 5 children or in a group with two teachers. If concerns continue, that child will need a parent or guardian chaperone to come on the field trip. Name-to-face counts will be conducted when exiting the vehicle and entering the vehicle as well as frequently throughout the duration of the field trip.
