Plan to Correct

Staff will engage with children, move about every area the children are in, classrooms, gym, large motor and outside. They will be consistent in monitoring the children, their environments, behaviors, and activities.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Plan to Correct

"Teaching staff are to be up moving around the classroom, gym, playground and large motor room. They are to be interacting with the children. Staff are not to be fixed in any position for any amount of time. Staff are to remain at a close distance to all children. Management staff will monitor teaching staff to ensure this is being followed through."

"Director terminated the employee in question. Director has printed off behavior and guidance materials and training for all staff. Director will ensure all staff are signed up for a behavior and guidance training by 4/1/24. Director will provide additional oversight of classrooms by stepping into classrooms and using the camera access to ensure staff are not using any inappropriate discipline on children. Director will ensure staff are using redirection with children without any mistreatment by having a staff meeting to ensure all staff are on the same page using appropriate discipline, as well as additional director oversight in classrooms."

340:110-3-288(c)(1)(A) - actions that could cause pain, such as shaking, hitting, striking with an object, spanking, swatting, thumping, pinching, popping, shoving, spatting, hair pulling, yanking, and slamming;

Plan to Correct

"Staff are only allowed to use their phones for documentation for procare. Director will meet with staff and go over cell phone policy. Director will monitor classrooms closely for phone usage. If it is see I will ask to see the phone to ensure it is for procare only."
