The program does not have documentation of having completed the physical environment checklist in the past 12 months.
The program has not completed an equipment inventory in the past 12 months.
The 2-yr-old and older classroom does not have interest areas for four required categories - books and language, blocks, manipulatives and dramatic play; the younger 2-yr-old classroom does not have interest areas for three required categories - books and language, manipulatives and art. The shelves and spaces for the interest areas are in place but equipment is lacking.
Ten teaching staff do not have current CPR/1st aid training; the training certification for these staff has expired; one staff employed past 90 days has not obtained the training; the other nine staff have not renewed training.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Supervision - Two children left the outdoor play area through a gate and were found in front of the center; staff in charge were unaware of the children's whereabouts.

Notice to Comply received 10/10/24 - On the day of the incident, the program director spoke with the two teachers in charge about making sure the gates for the outdoor play area were closed properly; the program has since enacted a plan to ensure proper supervision; management spoke with the two staff regarding supervision requirements for outdoor play; staff are now walking throughout the entire outdoor play areas to ensure the safety of all children; all staff will be participating in training on10/14/24 during which expectations for supervision will be reiterated; in addition, a work order has been made for all gates and latches to be checked to ensure they are working properly; in the meantime, management will observe all gates and latches each morning to ensure they are working properly.
Regulation Description
340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Notice to Comply received 2/7/25 - Management was retrained on licensing requirement of parents being notified of child incidents the day of the incident. New management is also in place.
