There is one staff member who has an expired PDL since 4/9/26.
In the 2 & older the evacuation route was not posted.
In the 2 and older classroom there were no books for the children to look at and not enough blocks for the amount of children in the classroom.
The little couch for children in the 2's and up room had rips in the seating area.
Crayon marks are present on the dramatic play toys in the 2 and older classroom.
In the classroom the toys were observed to be soiled with dirt and needing to be cleaned.
The lesson plans posted in the 2 and up classroom was dated 5/1 to 5/15.
The thermometer in the kitchen was not working so could not verify the correct temperature. There was no thermometer in the infant classroom.
Parent permission forms for all diaper rash creams located in the center cannot be verified on premises today.
Multiple classrooms lack play equipment quantity and variety in art & dramatic play.
Two diaper rash creames located in the ones classroom was not labeled with child's name
Three light sensor outlets in three different classrooms had electrical components exposed (4's classroom, ones and older classroom, and all 1 classroom).
Diaper rash cream was accessible in two classrooms with unlocked cabinets under the diaper changing table (ones and older classroom & all 1 classroom)
Three teachers does not evidence of having renewed their CPR/FA certification.

All staff members are required to undergo re-training on positive behavior support. The re-training will be divided into two parts, scheduled to take place during the staff meetings in June and July. The staff meeting for June is set for June 24,2025, while the meeting for July will be held on July 15, 2025.
