All staff will be re-trained on the importance of prompt and consistent documentation of diaper changes, feedings, and sleeping times. This will be done verbally to all infant and toddler teachers immediately and to entire staff at next staff meeting on 11/7/23.

340:110-3-281.4(b)(2) - Infant and 1-year-old daily record. Daily records for 1-year-olds and younger are available in the child's classroom and include prompt documentation of eating, diapering, and sleeping activities.

Plan to Correct

During floor play, infants will be better supervised including staff being close enough to intervene as needed and separating those infants mobile enough move themselves onto another child, pulling at or scratching them, etc. Written observations in class and by video will be conducted. Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

A designated eating area has been set up for all highchair eaters. Stackable high chairs will be used to safe floor spaced. Infants who are able to hold their own bottle will be placed in a high chair in designated eating area. Daily observations by video and in person will be conducted by director and assistant director.

Parents will be notified by written ouch report and verbally at pick up or by phone of any minor injury of mark.

Timers will be bought and used when a child is in a swing or other stationary equipment so that the infants are not in the equipment longer than 20 minutes. In class and by video observations by director and assistant director.

340:110-3-289(j)(3) - do not remain for more than 20 minute increments in equipment restricting freedom of movement, such as rest equipment, swings, high chairs, or stationary activity centers, except while eating in high chairs or similar stationary equipment. Children only sleep in appropriate rest equipment, per OAC 340:110-3-296.

Staff will be retrained on proper handwashing procedures for themselves and children. The importance of good hand hygiene will be stressed. Staff will sign that they have had this retraining. More detailed training will be provided in the future for new employees orientation. This will begin immediately. In class and by video observations by director and assistant director.

Regulation Description

340:110-3-294(a)(2) - Hand hygiene. Personnel and children wash their hands with soap and warm, running water or use hand sanitizer as required per Oklahoma Administrative Code (OAC) 340 Appendix HH – Hand Hygiene.

Any infant younger than 6 months old will always be held to drink their bottle. This includes infants older than 6 months but not able to hold the bottle themselves.
