Red chair has torn plastic, slide on swing set unattached to A-frame brace, one pack & play has a softball size hole in mess towards top.
Napping mats are missing sheet; blankets are within reach.

SERIOUS: Transportation - Child riding in car without proper child passenger restraint system.
Plan to Correct
I will make sure all kids will be in the proper car seat and restained correctly.
Regulation Description
340:110-3-87(e) - Passenger restraints and seating. Children remain properly secured in a child passenger restraint system, such as car or booster seat or individual seat belt, per Section 11-1112 of Title 47 of the Oklahoma Statutes (47 O.S. § 11-1112). Children do not share a seat belt.
Complaint:
SERIOUS: Transportation - Vehicle exceeded the manufacture's capacity.
Plan to Correct
Moving forward I will make sure not to exceed over the amount of kids that can be in a vehicle safely.

340:110-3-87(e)(5) - Vehicle maximum capacity and seating space, per manufacturer's designations, is not exceeded.
