CCLS observed that caregiver has been employed since 11/1/2001 and has not obtained a current OPDL as of 7/24/23.
CCLS observed that primary caregiver who has been employed since 11/1/2001 has not completed 12 clock hours annually and currently has 0 hrs to date.
CCLS observed that the fire extinguisher had not been replace or had a new tag added within the three year period as required and had expired on April, 2023.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.
