Non Compliance Observed Licensing observed three children's files without insurance notification forms. Numerous, Repeated, and/or Serious Non Compliance No Numerous: No Repeated: No Serious Non Compliance: No Plan to Correct Ms. Lambert printed copies of the form and began prepping the forms for parents to fill out and sign. Ms. Lambert will have parents fill out the forms upon child pick up and drop off. Ms. Lambert put the form in her child enrollment packet. In the future, Ms. Lambert will ensure parents fill out the entire enrollment packet prior to beginning childcare and annually in October. Regulation Description Parents complete DHS forms every 12 months regarding insurance liability and the compliance file. Forms are maintained at the facility. ¢ 6 Plan to Correct Date 2026-07-25

340:110-3-281.1(e)(2) - Allergy list has the children's classrooms listed. 340:110-3-279(d)(5)(B) - First aid kit in the vehicle has a cold pack, liquid soap, hand sanitizer, and a plastic bag. Licensing observed a staff member compassionately caring for a child during nap time who was having a hard time falling asleep. The staff member remained patient, kind, caring, and remained with the child until the child feel asleep. The other staff member maintained supervision of the other children.
