SERIOUS Licensing viewed a child in office hallway, peaking around the door way. Five year old child was allowed to go to the bathroom by himself, but left the bathroom without the teacher's knowledge. The pantry door was unlocked and open, leaving chemicals accessible to children.

Licensing brought the unsupervised child back to their classroom. Teaching personnel stated they will have their whole class use the girls restroom, which is within vision of the classroom hallway. Office assistant states they will print a paper stating doors must be locked at all times, posted on the pantry door. Office assistant states they will discuss a plan of correction with the Director, on how to prevent un-supervision from happening at the facility.

Licensing observed two staff members in the break room, during operating hours. Licensing saw evidence of light hazy smoke surrounding the staff member and a sweet unnatural smell. Licensing saw a vape sitting on the table, the staff member immediately grabbed the vape and put it in her pocket. When Licensing asked her about the item in her pocket, she confirmed it was a smoking vape.
