Non Compliance Observed Licensing observed that assistant caregivers PDL expired on 2/18/2025. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Provider states that assistant is working on getting his CECPD account back open. Provider stated that she will have assistant get this done ASAP. In the future provider stated that this will be completed on time. Regulation Description Assistant caregivers obtain and maintain a current Level 1 or higher OPDL certificate, per OAC 340 Appendix FF – Oklahoma Professional Development Ladder, prior to or within 12 months of employment. Professional development requirements
Non Compliance Observed Licensing observed that assistant caregiver has not completed any professional development over the last countable year. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Provider stated trainings have been completed but have not been able to upload due to assistant being locked out of account. Provider states that she will ensure that assistant logs into account and uploads trainings that have been completed and in the future will make sure this is done in a timely manner. Regulation Description The assistant caregiver is required to complete completes 12-clock hours of professional development annually. Plan to Correct Date 2025-06-14

Prior visit had non compliance of dogs not having rabies shots-this has been corrected and all animals are up to date on shots.
